CWP Application for Wellness Professionals

A Certified Wellness Practitioner application for a wellness professional includes:

  1. official transcripts documenting your academic preparation, degree(s) and educational background.
  2. a resume/curriculum vitae providing evidence of at least five years of full-time experience in the wellness/health promotion field demonstrating that wellness/health promotion programming is considered your primary work activity and is a direct responsibility in your job description. Include job descriptions and names (along with telephone numbers and emails) of references who can speak to your wellness leadership.
  3. a personal statement of qualifications describing your wellness leadership activities and continuing education and scholarship (750 to 1,000 words)

Application Fee
Along with the completed application (academic transcripts, curriculum vitae/resume, personal statement of qualifications), applicants must submit an application fee of $350* ($50 non-refundable) payable to the National Wellness Institute. The term of certification is five years and automatically includes a five-year membership in the National Wellness Institute (NWI) at no additional charge (currently, a $495 value). If your application is not approved, you will receive a refund of $300.

Maintaining Certification
Renewal of the CWP designation after five years requires documentation of 50 hours of continuing education and a $350* recertification fee (which includes another 5 years of membership in NWI). The re-certification process must be repeated every five years.
